If you are completing a student rotation with Harbin Clinic you will need to complete the below approval process:

1. Complete the online Georgia Statewide AHEC Network Support & Profile Form
   
2. Print, sign, and fax the Confidentiality Statement to Sharn Vericella at 866-602-0305
3. All mid-level providers must read the Harbin Clinic Guidelines.
