Instructor Requirements:

If you are an instructor sending students to Floyd Medical Center please complete the below instructor requirements. 

1. Instructor Profile Form
   Instructors should complete the basic Instruction Profile form before engaging in educational opportunities at Floyd. Once complete click submit and it will be sent electronically.
   
   
2. Instructor Security and Confidentiality Statement
   Review, print and sign the Security and Confidentiality Statement. Fax to Michelle Evans, Health Professions Liaison at 866.521.1542.
   
   
3. Watch the online Floyd Orientation Slideshow Presentation and then fulfill Floyd’s education requirement by completing the online Non-Employee Orientation Quiz and submit a passing score of 85. The orientation quiz consists of 50 questions. Please allow 30 - 60 minutes to complete the course.

• Affiliation Agreement
  Educational Institutions must have a current affiliation agreement with Floyd before their students can receive approval to do their clinical rotation. Schools can fax their agreements to Michelle Evans, Health Professions Liaison at 866-521-1542.
• Rotation Request Form
  Complete this form to request placement for a student rotation.
• Student Rotation Requirements Summary
  Instructors are required to complete this form for all students who rotate at Floyd Medical Center.
• Immunization Requirements
  This form provides details about the immunizations required for all students.
