Physician student rotations give students the opportunity to study health care professionals as they work in a health care environment while under the supervision of a Preceptor. Rotations or clerkships are arranged by your medical program.
- Complete the electronic Floyd Medical Student Profile Form.
- Complete the online Rotation Request form if one has not been provided by your school.
- Watch the online Floyd Orientation Slideshow Presentation and then fulfill Floyd’s education requirement by completing the online Non-Employee Orientation Quiz and submit a passing score of 85. The orientation quiz consists of 50 questions. Please allow 30 - 60 minutes to complete the course.
- Download and complete the Physician Student Rotation packet.
- Obtain a copy of a current Background check and immunization records, unless on file with your college or university.
- Fax completed Physician Student Rotation Packet, immunization records, and background check to Floyd Medical Center at 866.521.1542. If you have any questions call Michelle Evans, Health Professions Liaison at 706.509.5136 or cell phone 706-676-8797.
You and/or your instructor will be notified by Michelle Evans, Health Professions Liaison when your requirements have been fulfilled and you have received approval.
Once approval has been received, you will need to go to Floyd’s Security Department, located on the 1st floor of Floyd Medical Center (directly across from the pharmacy), 304 Turner McCall Blvd., Rome, GA 30165, to obtain your student badge. They are open Monday-Friday from 8am-12pm and 1pm-4pm.
